Get your LOA approved before your visit

Request HMO pre-approval online before your visit. Our HMO Help Desk coordinates with your HMO, and your Letter of Authorization (LOA) appears in your account once approved.

How it works

    Submit your request

    Choose your HMO, enter your member details and the service you need.

  2. 2

    Upload your documents

    A clear photo or PDF of your HMO card, valid ID and doctor's request.

  3. 3

    We coordinate with your HMO

    Our HMO Help Desk checks your documents and contacts your HMO. We may ask for more information.

  4. 4

    Receive your LOA

    We notify you of the decision. Your LOA number, coverage and validity appear in your account.

Documents usually needed

  • HMO card (front and back)
  • Valid government ID
  • Doctor's request / prescription

Some HMOs need additional documents. Approval and coverage are decided by your HMO. An approved LOA may cover only part of the cost — any amount not covered is paid at the hospital.
